Thank you for choosing EverTrace Testing Services LLC for your DNA collection and testing needs. We are committed to providing professional, confidential, and accurate services. Please review the following information to understand our policies and procedures.
Policies
1. Scheduling & Appointments - Appointments must be scheduled in advance by phone, email, or online booking. - Same-day or expedited appointments may be available at an additional fee.
2. Payment - Payment is due at the time of booking. - Accepted methods: debit/credit, online payment]. - All sales are final. Refunds are not issued once testing has been conducted.
3. Cancellation & Rescheduling - Appointments canceled with less than 24 hours’ notice are subject to a $50 cancellation fee. - Missed appointments without notice are non-refundable.
4. Confidentiality & Privacy - EverTrace strictly adheres to confidentiality standards. - Test results are released only to authorized parties in writing. - Personal data is stored securely and not shared without consent.
5. Legal & Non-Legal Testing
- Clients are responsible for ensuring they select the correct type of test (legal vs. non-legal).
- Court-admissible testing requires verified chain of custody procedures.
6. Limitations & Disclaimer
- EverTrace Testing Services LLC is a collection service. Laboratory testing and results are processed by accredited partner laboratories.
- We are not responsible for laboratory turnaround times, errors, or disputes related to interpretation of results.
7. Test Limitations & Possible Outcomes
- In rare cases, a test may be inconclusive, contaminated, or unsuccessful due to: • Insufficient DNA sample • Contamination (food, drink, smoking, etc. before collection) • Low-quality or degraded sample - If a sample is deemed unsuitable, the laboratory may request a new collection.
- Re-collection may result in additional fees unless otherwise stated at the time of service.
- EverTrace is not responsible for delays or costs caused by unsuccessful samples.
8. Mother’s Participation (Strongly Encouraged)
- For paternity testing, the mother’s participation is highly encouraged.
- Including the mother’s DNA sample can: • Strengthen the accuracy and conclusiveness of the test • Eliminate potential ambiguities in the results • Speed up processing time at the laboratory
- While not always required, the mother’s participation helps ensure the most reliable outcome.
- Important: If the laboratory later determines that the mother’s sample is required and she was not originally included, additional testing fees will apply for re-collection and processing.
- If the mother participates from the beginning, there are no additional fees for her inclusion.
9. Collection Procedures & Security - For the safety and privacy of all parties, EverTrace does not collect samples from all parties together.
- A guardian and child may be collected together, but alleged fathers are always collected separately.
- Once the collection process has begun for any party, no refunds will be issued, even if the alleged father or another party fails to appear.
10. Release of Test Results
- Results are released only to the paying/authorizing client who ordered the test.
- EverTrace will not provide results, proof of collection, or chain-of-custody documents to non-paying parties.
- Legal/court-admissible tests are strongly encouraged over peace-of-mind tests because they include: • Verified chain of custody • Government-issued IDs collected for all tested parties • Documentation proving who was tested - Choosing a peace-of-mind test means the client accepts responsibility for any disputes regarding the identity of tested individuals.
11. Results Distribution & Multiple Paying Parties
- Results are released only to the paying/authorizing client.
- If you require results sent to more than one paying party, do not book online.
- You must call our office directly to arrange this request.
- Each additional paying party will be sent a separate invoice to split the cost of testing before results are released.
- Testing starts once all invoices are paid.
Procedures
Before Your Appointment
- Bring a valid government-issued photo ID.
- Ensure all individuals being tested are present (unless prior arrangements are made).
- Refrain from eating, drinking, smoke, chewing gum or tobacco 1 hour prior to sample collection.
- Babies should not be breastfed 2 hours prior to appointment.
- Follow all instructions carefully to reduce the risk of an unsuccessful sample.
During Collection
- A certified collector will verify IDs, obtain consent, and document chain of custody (if applicable).
- Samples are collected using sterile buccal swabs or as otherwise required.
- The process typically takes 15–30 minutes for each party.
After Collection
- Samples are securely packaged and shipped to an accredited laboratory.
- Standard turnaround time: 2-3 business days.
Expedited services may be available.
- If the laboratory reports a failed or inconclusive result, clients will be notified immediately with options for re-collection.
- Final results will be delivered directly to the authorized contact by (email or mail).
